The present study aimed to evaluate the therapeutic effects of liver transplantation on cHCC-CC and analyze the clinicopathological factors affecting prognosis. BACKGROUND/AIM: Hepatic tumors with elements of both hepatocellular and cholangiocellular carcinoma are occasionally encountered; however, the independent and simultaneous occurrence of different epithelial malignant tumors in different lobes is rare. This is a case report of resected double cancer in different lobes of the liver. A few cases involving both hepatocellular and cholangiocellular components in the same tumor have been designated as ‘combined hepatocellular and cholangiocarcinoma (cHCC-CC),’ and 1.0–4.7% of such cases have been reported among primary liver cancer patients (1– 7). Double primary liver cancer, i.e., hepatocellular carcinoma (HCC) and cholangiocellular carcinoma (CCC), was detected by ultrasound (US) during the follow-up of chronic liver disease, and was safely resected in two patients, one synchronously and the other metachronously. Separate hepatocellular and cholangiocellular carcinoma (double cancer) in the liver are extremely rare subtypes of primary hepatic carcinomas. We report a case of double primary liver carcinomas that were surgically resected simultaneously.

Although mixture of two types of cellular components in one mass has been reported and well documented, there are patients with two carcinomas concurrently and separately [ 1 - 4 ]. Some hepatocellular carcinomas recognisably resemble hepatocytes, or produce identifiable bile. But pseudoglandular formation, clear cell change and poor differentiation may make this difficult. The tumours which need to be differentiated include cholangiocarcinoma, combined hepatocellular and cholangiocarcinoma and metastatic carcinomas. One distinct, albeit rare, subtype of HCC is combined hepatocellular-cholangiocarcinoma (cHCC-CC), which overall carries a poorer prognosis than HCC and cholangiocarcinoma (CC) alone.
